2.  Complete page 1 of the form with your name, address, phone number, and email address
3.  Complete your payment information
4.  Enter your name, date, and the quantities you would like to order on page 2
5.  Save the form to your computer
6.  Email your completed form to our supplier FX Med. Their email address is: support@fxmed.co.nz
7. FX Med will send the supplements out to you directly
8. Once FX Med have your prescription on file, you can simply order each time by emailing them
